2. *A brief description of the nominee's organization including description of the products, services, overall strategic goals
3. *Last 2 years annual report of the company and organization chart
4. *Supporting documents of the various evaluation parameters as mentioned in the nomination form.

1. Details of membership of any other professional/social bodies
2. Details of other roles such as directorship/ member of audit committees of other companies etc.
